In January 1997, three established Family Practice physician groups in Frederick County merged to form Frederick Primary Care Associates (FPCA). Realizing the need to serve the southern Frederick City area, FPCA purchased land on Solarex Court, off Ballenger Creek Pike, and began construction of this office. The Ballenger Creek office was completed and opened in July 1997. Dr. Stephen Lee moved his practice from Thomas Johnson Drive to the new location.

All of the physician partners of FPCA contributed to the effort to successfully open and grow the Ballenger Creek office. Dr. Lee was joined in the practice by Dr. Jill Durfee in 1999. Dr. Dale Heitzig moved to the office from FPCA’s Woodsboro office in 2000. Most recently, Dr. Vivian Dechosa joined the office in 2009.
